Data Controller Application Form

1. General Information

Law No. 6698 on the Protection of Personal Data (“KVKK”) grants the data subject (“Applicant”) the right to make requests regarding the processing of their personal data under Article 11 of the KVKK.

The application has been prepared to determine your relationship with Melekırmak Sağlık Ürünleri Taşıma Gıda İnşaat Sanayi Limited Şirketi (“Melekırmak”), to determine completely the personal data, if any, processed by Melekırmak, to enable your application to be answered correctly and within the statutory time, and to make the process of applying to the data controller easier for you.

In order to ensure the security of your personal data and prevent unlawful data transfer, Melekırmak may request additional information for identity and authority verification. If the information stated by the Applicant is incorrect and/or out of date, or if the requests are unauthorised, the responsibility in this regard lies with the Applicant.

Pursuant to Article 7 of the Communiqué on the Procedures and Principles of Application to the Data Controller, if the Applicant's application is to be answered in writing, no fee is charged for up to ten pages. A processing fee of 1 Turkish Lira may be charged for each page over ten pages. If the response to the application is given on a recording medium such as a CD or flash drive, a fee equal to the cost of the recording medium may be requested.

2. Scope of the Right of Application under Article 11 of the KVKK

By applying to Melekırmak, the Applicant may make requests on the following matters:

1. To learn whether their personal data is processed,
2. To request information if their personal data has been processed,
3. To learn the purpose of processing their personal data and whether it is used in accordance with that purpose,
4. To learn the third parties, in Turkey or abroad, to whom their personal data is transferred,
5. To request the correction of their personal data if it is incomplete or inaccurate, and to request that the action taken in this context be notified to the third parties to whom their personal data has been transferred,
6. To request the erasure, destruction or anonymisation of their personal data if the reasons requiring its processing no longer exist, even though it has been processed in accordance with the KVKK and other relevant legal provisions, and to request that the action taken in this context be notified to the third parties to whom their personal data has been transferred,
7. To object to the occurrence of a result against them by analysing their processed data exclusively through automated systems,
8. To request compensation for the damage if they suffer damage due to the unlawful processing of their personal data.

3. Method of Application

Pursuant to the first paragraph of Article 13 of the KVKK, applications regarding these rights must be submitted to us in writing and signed, or by other methods determined by the Personal Data Protection Board (“Board”).

In this context, applications to be made in writing may be submitted to us by printing this form;

• By the Applicant's personal, in-hand application,
• Through a notary,
• By registered electronic mail (KEP) address, secure electronic signature, mobile signature, or
• By e-mail sent from the e-mail address that the Applicant previously notified to us and that is registered in our system.

Below, information is given on the written application channels, on how written applications will reach us.

In applications made by the Applicant by e-mail, Melekırmak may request additional information to verify the Applicant's identity and may take the necessary measures.

Pursuant to the second paragraph of Article 13 of the KVKK, the applications submitted to us will be answered in writing or in electronic form within thirty days from the date the request is notified or reaches us, depending on the nature of the request.
